To update details like your name, address, phone number or email address:
- in your NHS account (through the NHS website or NHS App)
- in your Patient Access account
- use our eConsult service
- phone us on St Andrews Medical Centre 0208 459 7755 or Neasden Medical Centre 0208 208 0306 or Greenhill Park Surgery 0208 838 3100 or visit the surgery from 10am to 6pm
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
