Social Prescribers

If you are a patient or a carer of a patient who you think would benefit from being referred to the social prescriber, please let your GP practice know. Social Prescribers are attached to the practice and can give you confidential advice and help support you not just with accessing specialist services and support but during the coronavirus pandemic, are able to carry out telephone assessments of your needs such as access to food, prescriptions and emotional support.
